The following procedures outline the process for becoming a member of the Mood Field Network:

1.  Fill out our online survey.  (Link will open in a pop-up window)

2.  Contact your regional Field Operations Manager.

3.  Submit the following documentation to your regional Field Operations Manager:

• Signed IRS W-9.
• Signed Master Subcontractor Agreement
• Certificate of Insurance meeting or exceeding Mood’s insurance requirements as noted in item #11 of the Master Subcontractor Agreement.
